LONG CASE
This is an online e log book to discuss our patient’s de- identified health data shared after taking his/her/guardian’s signed informed consent. Here we discuss our individual patient’s problems through series of inputs from available global online community of experts with an aim to solve those patient’s clinical problems with collective current best evidence based inputs. HOPI : patient was apparently asymptomatic 2 years back then he developed jaundice
After many attacks he visited our hospital
And developed edema in lower limbs pitting type insidious in onset gradual in progression and then he developed swelling in abdomen insidious gradual progression
h/o nausea ,tiredness after eating
h/o sleeplessness from 2 weeks
h/o nervousness in hands
Tingling sensation in hands
Past history:
N/K/C/O : DM, Asthma , epilepsy, thyroid disorders
Medication was done- few fluid(30 ml)removed from abdomen
Personal history:
Sleep :inadequate
Bowel movement: regular
Addiction:
14% alcohol
375 ml
Weekly 5 times
alcoholic (stopped from 2 months )
Family history
No history in family
